1、骨科系申報編碼作業骨科系申報編碼作業 病歷管理組病歷管理組 黃素勤黃素勤 財團法人嘉義基督教醫院大綱大綱骨骼、肌肉系統疾病分類規則概要疾分編碼與DRG之相關性關節病變關節病變代碼範圍：710-719闗節炎(arthritis)：是造成關節發腫、难过、僵硬等毛病的總稱，而較常見的有骨性關節炎(osteoarthritis；OA)類風濕性關節炎(rheumatoid arthritis)痛風性關節炎(gouty arthritis)關節病變關節病變骨性關節炎骨性關節炎(osteoarthritis；OA)(715.XX)局部骨性關節炎原發性(primary)：715.1x續發性(secondary
2、)：715.2x局部性關節炎(localized arthritis)：715.3x 當醫師有明示部位但無提及為原發或續發性時以715.3X(包含雙側同一部位)多發部位關節炎：715.8X關節病變關節病變例：例：osteoarthritis，bilateral，knee Ans：715.36例例：osteoarthritis of knee and hip Ans：715.89關節病變關節病變外傷性關節炎(traumatic arthritis)：716.1x是因關節受傷後所引起之關節炎 例：例：traumatic arthritis，left ankle，due to old traumat
3、ic dislocation Ans：716.17+905.6(late effect)+E929.X關節病變關節病變 類風濕性關節炎(rheumatoid arthritis)：714.0 類風濕性關節炎是一種免疫上的疾病，會造成 關節的慢性發炎，有時也會侵扰關節的周圍組 織或者是身體其他器官，症狀主要有關節紅、腫、熱、痛。關鍵字：arthritis關節病變關節病變痛風性關節炎(gouty arthritis)：274.0 痛風是一種身體內普林（Purine，或稱嘌呤）代謝異常的疾病，使血液裡尿酸濃度過高，醫學上稱為高尿酸血症，臨床上主要症狀為急性關節炎發作。痛風石(tophi)：274.8

10、Back pain not otherwise specified：724.5Psychogenic back pain：307.89若確知造成back pain之缘由，此back pain是屬於該病因之病徵時，則不再加編背痛之代碼。背部病變脊椎狹窄(spinal stenosis)：723.0 或724.xx 黃韌帶肥厚，小面關節長骨刺及其囊肥厚，椎間盤突出访得dural space減少 due to 以下兩種狀況,則723.0 or 724.0 x可 省略不編碼 (1)HIVD(722.xx)(2)spondylosis(721.x)若無due to,則spinal stenosis 不行
