2、姓名Name职务Title说明：申请表由机构法人代表或已在引智司备案的被授权负责人签字，重要信息变更需附有关证明文件，并由引智司视情征求驻外科技处/组或驻在国使领馆相关部门意见。请将信息变更申请原件寄至科技部引智司出国培训二处。Notes: This application form shall be signed by the Legal Representative or the authorized person in charge who is already recognized by the Department of Overseas Intellectual Resources
3、 Cooperation of MOST. For change of important information, certification documents are required to be attached, and if necessary, our Department may consult S&T Section or relevant departments of Embassy/ Consulate-General of P.R.C in the host country. The original copy shall be mailed to Division of Overseas Training of our Department. 1
