1、讲课费协议书 Contract with Speaker 甲方 Party A：浙江大冢制药有限公司 Zhejiang Otsuka Pharmaceutical Co.,Ltd.乙方 Party B：先生/女士，自 医院（或其他医疗机构）职务 Mr./Mrs.from Hospital(or other medical institution)Title 该合同由浙江大冢制药有限公司（以下称为甲方）与 （以下称为乙方）签定。甲方邀请乙方，乙方也在此接受甲方的邀请作为讲者在中华人民共和国的 市，于 日为 产品就有关双方共同认可的 题目进行演讲，对于 产品功能的描述，应确保在药监局发给该产品的批
2、文规定的范围内。乙方保证其接受甲方之邀请作为讲演者一事，没有违反其所属医院/医疗机构的任何规定或文件、也没有违反其签署过的任何合同、承诺书或其他文件，否则乙方将承担由此造成的所有损失。如果乙方为提供演讲而需离开所居住城市，由此产生的抵达及离开会议地点的旅行与住宿费用由甲方承担。乙方将在协定的日期就共同认可的题目进行演讲。如时间有变或无法按时演讲应在合理的时间内通知甲方。甲方将在演讲后为乙方提供（税前/税后）的讲课费。乙方在接收时应签字并提供个人身份证复印件为甲方代缴其个人所得税之用。乙方若是外籍人员，需提供护照、签证、资质证明复印件。This contract is signed betwee

5、ty Bs travel cost to and from the meeting venue and accommodations occurred for his/her lecture during the meeting time if he/she needs to travel out of his/her home city for lectures purpose.Party B will give the lecture on the agreed date and topic.Any change of the date or inability to give the l
6、ecture shall get Party A to be informed before a reasonable time .Party A will provide Party B an amount of(Before Tax/After Tax)as speakers honorarium after the lecture.Party B shall acknowledge the receipt of the honorarium upon receipt by signing the receipt letter as well as the copy of personal
