1、附件产品认证申请书Application Form for Product Certification 申请人/Applicant 填写申请企业的名称 申请日期/Date 申请认证产品名称/Product(s) to Be Certified 填写大类名称（如机动车灯具，机动车后视镜等），名称应与产品所适用的实施规则上的产品名称一致 中汽认证中心China Certification Centre for Automotive Products以下内容由认证机构填写(The following is for CCAP use only)：申请书编号(Application Form No.)：
2、 证书编号(Certificate No.)： 工厂代码(Factory Code)： 证书流水号(Certificate Sequential No.)： 填 表 说 明Instruction for the Application Form1、请下载此表格后用电脑输入，若手写请使用钢笔或碳素笔，字体工整字迹清楚.Please try best to download this form and use compute to input information. In the case of filling by hand, please complete all items with a
3、black pen neatly and clearly.2、表栏空格不能空白，无此项时应划“/”线Please do not leave any items blank, fill in “/” with the items which are not applicable.3、“申请方承诺”是认证中心根据国家产品认证有关法律、规范及标准等拟定,是申请方应尽的最基本的义务“Promises from applicant” is constituted by CCAP according to relevant laws, criterions and standards etc. of na

7、： 2.3 联系人/Liaison person： 电话/ Tel ： 传真/Fax： 电子邮件/E-mail： 网址/Web site： 3. 制造商/Manufacturer (请附营业执照和组织机构代码证书。Please affix the business licence and )3.1名称/Name: 此处填写制造商名称，制造商不一定是生产该产品的企业，但必须 是承担设计和制造责任，并能够控制生产的产品与检验样品一致 3.2地址和邮编/Address and post code:制造商的地址要与递交的工商营业执照上的地址完全一致 3.3 联系人/Liaison person：联系人
8、及联系方式(包括申请人和制造商不一致时) 应准确、便于联系 电话/Tel ： 传真/Fax： 电子邮件/E-mail： 网址/Web site： 4. 工厂/Factory(请附营业执照和组织机构代码证书。Please affix the business licence and )4.1名称/Name: 请填写中/英文名称， 须与工商营业执照一致。有多个工厂的请分别填写 4.2地址、邮编/Address and post code: 总部与生产厂不在一起的，填生产厂地址，须与营业执照一致 4.3 联系人/Liaison person：请填写生产厂的联系人，联系方式应准确、便于联系 部门及职位
9、/Department/position： 电话/Tel ： 传真/Fax： 电子邮件/E-mail ： 网址/Web site： 5. 认证产品的型号名称，请填写附表认证产品型号汇总表/The model and name of products to be certified, please fill in appendix: Compile Form of Models to be Certified 申请认证产品的具体名称，譬如汽车前照灯；转向灯；液压制动软管；气压制动软管等 6. 认证产品注册商标/Registration trademark of the product to be
10、 certified 如是图形，请附在本表后面。同时将电子版发送到CCAP的邮箱 7. 申请认证产品适用的国家或行业标准/ GB/QC standards for the product(s) to be certified7.1 国家标准/GB Standard （可另加附页） 填写标准号和标准名称 7.2 行业标准/QC Standard 同上 8申请人希望将产品检验时间安排在 年 月; 将工厂质量保证能力审查时间安排在 年 月为宜。Applicant would like the product inspection to be carried out in _(Y) _(M) and
11、the factory quality assurance ability inspection to be carried out in _(Y) _(M).说明：表中各条款的黑体字，为申请人填写时应注意的说明，申请人填写时将说明去掉。申请方承诺/Promises from applicant1、始终遵守认证安排及有关规定；We will comply with certification plan and relevant regulations through out the certification period.2、为进行评价做出必要的安排，包括审查文件、进入涉及认证产品有关的区域
12、、审阅有关记录（包括内部审核报告）和为评价（例如检验、检查、评定、监督、复评）和解决投诉准备相应的人员；We will make necessary arrangements for assessment, involving inspecting documents, entering all areas relevant to the products to be certified, checking relevant records (including internal auditing reports) and prepare relevant personnel for eval
13、uation (e.g. testing, inspecting, assessing, supervising and reviewing) and solving relevant accusations.3、在获证产品的范围内做出有关的声明；We will only make relevant announcement within the limitation of certified products.4、使用产品认证结果时，不得损坏CCAP的声誉、不得做CCAP认为可能误导或未经授权的声明；We will not stab CCAPs reputation and make any
14、 announcement which CCAP may think they are misleading or unapproved, while using the product certification results.5、当认证证书被暂停或撤销时，应立即停止认证证书和认证标志的使用，以及涉及认证内容有关的广告，并按CCAP要求交回所有的认证文件；We will stop using the certificate, marks and involved advertisements immediately if the certificate is suspended or wi
15、thdrew. We will also return all certification documents back to CCAP as required.6、认证仅用于表明获证的产品符合特定标准；The certificate is only for showing that the certified products have met specified standards.7、当产品设计或产品规范的更改对产品质量产生严重影响时或供方所有权、组织结构、管理者、产品生产场所发生变化或有信息表明产品可能不再符合认证要求的情况下应重新进行评价；We will go through the
16、 certification procedures again if the changes in product design and product standards seriously affect the product quality or suppliers production rights, organizational structure, managers and producing place are changed or any information which shows the products may no longer meet the certificat
17、ion requirements.8、确保不采用误导的方式使用或部分使用认证证书和报告；We will make sure not to use or partly use the certificate or certification reports for the sake of misleading.9、在传媒中对产品认证内容的引用，应符合CCAP的要求。We will not use any quotations which are against CCAPs requirements in media from the contents of product certificati
18、on.10、我们声明我们将遵守国家有关法律法规及CCAP的规则和程序，支付认证所需的所有费用。We promise that we will comply with all relevant laws and regulations established by the China Government, regulations and procedures of CCAP.
