5、 Scope of Application1.1 目的 Purpose为建立、健全SIGMA职业安全卫生管理体系,确定职业安全卫生方针和目标,确保被全体员工理解、执行和保持,持续改进职业安全卫生管理体系的有效性,特编制本职业安全卫生管理手册。To establish and improve Sigmas occupation security hygiene management system, to determine the occupation security hygiene policy and aim, to make sure staff could understand, im

6、plement and maintain it, and to continuingly improve its effectiveness, our company particularly draws up the “occupation security hygiene management manual”. 1.2 适用范围 Scope of Application 本手册适用于SIGMA全体员工职业安全卫生及相关活动的管理。This manual is applicable to Sigmas management on its staffs occupation security

7、hygiene and related activities.2 相关文件 Related Documents EHS职业安全卫生管理体系规范EHS “ occupation security hygiene management system-norm”质量管理手册 “ quality management manual “3术语和缩写 Terms and Abbreviate3.1 术语 Terms本职业安全卫生管理手册及本司职业安全卫生管理体系中使用的职业安全卫生术语采用EHS中的术语。 All terms of the “occupation security hygiene mana

8、gement manual “and our companys occupation security hygiene management system are adopted from the terms of EHS. 3.2 缩写 AbbreviateSIGMA本司 SIGMA Company4 职业安全卫生管理体系要素 Essential Factors of Occupation Security Hygiene Management System 4.1 总要求 General Requirement SIGMA根据EHS标准的要求建立、实施、保持和持续改进职业安全卫生管理体系。

9、为了实施和保持所建立的职业安全卫生管理体系,本司: According to EHS standard requirement, Sigma has established, enforced, maintained, and continuingly improve the occupation security hygiene management. To enforce and maintain the occupation security hygiene management system, our company would do the following steps: a).

10、制定适宜的职业安全卫生方针; Establishing suitable occupation security hygiene policyb). 识别职业安全卫生危险,以评估职业安全卫生风险;Recognizing danger of occupation security hygiene so as to evaluate it. c). 确定有关的法律、法规要求; Determining the requirement of related laws and regulations d). 建立适当的职业安全卫生目标; Establishing suitable occupation

11、security hygiene aim e). 建立组织机构,制定方案,以实施职业安全卫生方针,实现职业安全卫生目标; Establishing organization structure, setting up policy so as to enforce the occupation security hygiene policy and to fulfill its aim.f). 顺利开展策划、控制、监测、纠正措施、审核与评审活动,以确保对职业安全卫生方针的遵循和职业安全卫生管理体系的持续适宜性; Carrying out plan, control, monitoring, r

12、ectification steps, audit and check activities so as to observe the occupation security hygiene policy and maintain the system suitably. g). 根据不断变化的客观条件修正职业安全卫生管理体系。 According to the changeably objective conditions to revise the occupation security hygiene management system.4.2 职业安全卫生方针 Occupation S

13、ecurity Hygiene PolicySIGMA通过建立职业安全卫生管理体系,以持续改进职业安全卫生的管理和行为,达到保护员工职业健康和财产安全,以消除或减少职业安全卫生风险。为此,我们承诺: Through establishing the occupation security hygiene management system, SIGMA would continuingly improve our occupation security hygiene managements and behaviors so as to protect our staffs health an

14、d property as well as remove and decline the occupation security hygiene risks. We promise that:恪守法律法规,承担社会责任;改进安卫绩效,增强员工满意。 We would faithfully observe laws and regulations, shoulder society responsibility, improve achievements of security and hygiene, and satisfy our staff. 此职业安全卫生方针是SIGMA的职业安全卫生管

15、理工作纲领,本司通过发放职业安全卫生管理手册、组织各级培训等多种方式传达到每一员工,使员工理解并贯彻执行。 The occupation security hygiene policy is the working program of SIGMA. Our company would grant the “occupation security hygiene management manual” to each employee and organize all kinds of trainings so that all staff could understand and enforc

16、e it. 本司通过制定、分解、实施目标和实施运行控制来贯彻职业安全卫生方针。 Our company would formulate, explain and enforce plan as well as enforce circulation controls to implement the occupation security hygiene policy. 4.3 规划 Program 4.3.1 危险辨识、风险评估和风险控制规划 Program on Recognizing, Evaluating and Controlling risks 4.3.1.1 本司识别在生产、管理

17、活动中现存的和潜在的职业安全卫生危险, 从而进行风险评估和实施必要的控制措施。 Our company recognizes the existing and hidden occupation security hygiene risks through operation and management activities so as to evaluate risks and give necessary control steps. 4.3.1.2职业安全卫生危险的识别由人力资源部组织各部门进行,重大职业安全卫生危险由管理者代表确认。 The HR department takes c

18、harge of the recognition on the occupation security hygiene risks. The administrators confirm the great occupation security hygiene risks. 4.3.1.3职业安全卫生危险的范围包括: The Scope of the Occupation Security Hygiene Risks Includes:A.本部门生产、管理活动中的常规和非常规的活动; Each departments operation and management activities i

19、nclude conventional and unconventional activities.B.所有接近工作场所的人员(包括分包方和参观者)的活动; The activities of all people who approach the operation area (includes the subcontractor and visitors)C.工作场所的设施,无论是本司的还是其他组织提供的。 All facilities in the operation area includes both our own and that provided by other compan

20、ies.4.3.1.4识别方法及步骤Recognition Ways and Steps: A.人力资源部向各部门发放危险、危害因素识别排查表要求各部门识别出本部门职业安全卫生危险,在规定时间内上报给人力资源部,人力资源部根据收回的危险、危害因素识别排查表进一步审查,补充遗漏,并汇总登录在危险清单上; The HR Department would grant the “Form of recognizing and checking dangerous and harmful factors” to each department which are required to check ou

21、t its own occupation security hygiene dangers and reported to the HR Department in time. The HR Department would go a step further, replenish drops of the form and summarize all drops on the” danger list”.B.识别职业安全卫生因素与职业安全卫生危险按照以下原则: The recognition of the factors and dangers of the occupation secur

22、ity hygiene is according to the following principles:三种时态:过去、现在、未来 Three kinds of tense: past , present, future三种状态:正常、异常、紧急 Three kinds of conditions: normal, abnormal, emergency二十种类别:在平地上滑倒/跌倒;人员从高处坠落;工具、材料等从高处坠落;头上空间不足;与手工提升/搬运工具、材料等有关的危害;与装配、试车、运行、维护、改型、检修和拆卸有关的机械、设备的危害;车辆危害,包括场地运输和公路运输;火灾与爆炸;可吸

23、入的化学物质;可能伤害眼睛的物质或试剂;通过皮肤接触和吸收而造成伤害的物质;由于摄入引起伤害的物质(通过口腔进入人体);有害能量(如:触电、辐射、噪声、振动);由于经常性的重复动作造成的与工作相关的上肢损伤;不良的热环境,如过热;照明度;场地/地面易滑和不平;楼梯护栏不足;对员工的暴力行为;承包人的活动。 Twenty types: slipping and falling on the smooth ground; workers fall down from the high; tools and materials fall down from the high; insufficien

24、t overhead space; dangers related to hand hoist tool and handing equipment, material and so on; dangers related to assemble, test-drive, move, safe ground, modify, overhead, and dismantle machinery and appliance; vehicle dangers (includes the ground transport and road transport); fire and explosion;

25、 inhaled chemical matter; matter or reagent may harm eyes; matters cause harm through contact and absorb skin; absorb harmful matters (through oral to internal body); harmful energy (e.g. electric shock, radiation, noisy, vibration); upper limbs injury caused by regular and repetitive action or rela

26、ted works; heat surroundings (e.g. superheat); intensity of illumination; slippery or uneven ground; banister of the stairs is insufficient; violent behaviors to staff; contractors activities.4.3.1.5评估的方法和步骤Ways and Steps of Evaluation: A.人力资源部组织相关人员,对登录的危险进行评价。对于评价出的重大安卫风险,由人力资源部登录在重大安卫风险清单上; HR De

27、partment would organize relevant person to evaluate the recorded risks as well as record those evaluated great dangers on the “great security hygiene danger list”. B.根据其范围、特征和时限加以界定,以保证该方法是有计划的而不是临时性的; To define these dangers according to their scopes, characters, and time limit so as to make sure t

28、hese plans are well organized but not temporarily. C.对职业安全卫生风险进行评估,并辨识出哪些职业安全卫生风险可采用4.3.3和4.3.4中确定的措施加以消除或控制; To evaluate occupation security hygiene danger and recognize which of them could be declined and controlled by using the assured measure 4.3.3 and 4.3.4. D.与运行经验和采用的职业安全卫生风险控制措施的控制能力相一致; The

29、 circulation experience is in accord with the control abilities using on the occupation security hygiene control measures.E.为确定设备要求、确认培训需求和或建立作业控制提供依据; To provide bases for assuring the equipments requirements, training demands and work control. F.考虑到对所要求采取的措施的监测,以保证其实施的有效性和时间性;Monitoring the requir

30、ed measures so as to ensure its effectiveness and timeliness 4.3.1.6本司职业安全卫生危险随生产、管理活动的不断变化和法律、法规及其他要求的更新而不断进行评审和更新。 Our occupation security hygiene would be continuously examined and renewed with the changing of the operation and management activities as well as the updating of the laws, regulation

31、s and other requirements. 4.3.1.7职业安全卫生危险的识别和风险的评估按危险辨识和风险评估管理程序进行。The recognition and evaluation of the occupation security hygiene dangers would implement according to the” management measures of the recognition and evaluation about risks”. 4.3.2法律与其他要求 Laws and other Requirements4.3.2.1 本司根据已识别职业

32、安全卫生危险及时获得并识别适用于本司的职业安全卫生法律、法规和其他要求,从而为职业安全卫生风险评估、制定目标提供依据。 According to the recognized occupation security hygiene dangers, we would acquire and recognize occupation security hygiene laws, regulations and other requirements, which are suitable to our company so as to provide bases for evaluating, a

33、nd setting up aims for the occupation security hygiene dangers. 4.3.2.2人力资源部负责同职业安全卫生认证机构联系。人力资源部负责职业安全卫生法律、法规及其他要求的获取、更新和识别工作。The HR Department takes charge of contacting with the occupation security hygiene certification body as well as acquiring, renewing and recognizing occupation security hygie

34、ne laws, regulations and other requirements. 4.3.2.3人力资源部获取职业安全卫生法律、法规、标准及其他要求并确认其适用性,追踪新出台的法律、法规及其他要求。 The HR Department should acquire occupation security hygiene laws, regulations, standard, and other requirements and assure their applicability as well as pursue the new published laws, regulation

35、s and other requirements. 4.3.2.4 经确认适合于本司的职业安全卫生法律、法规及其他要求,人力资源部要建立法律、法规及其它要求清单,并根据法律、法规及其它要求的变化及时更新。The HR Department would establish the list of laws, regulations and other requirements, which are applicable to our company as well as renew them according to the change of these regulations. 4.3.2.

36、5各部门应遵守己确认的职业安全卫生法律、法规及其他要求。 Each department should adhere to the assured occupation security hygiene laws, regulations and other requirements.4.3.2.6 本司职业安全卫生法律、法规及其他要求的获取、更新和识别工作按法律、法规及其他要求管理程序进行。 The acquirement, renewal and recognition of the occupation security hygiene laws, regulations and oth

37、er requirements should implement according to the “management measure of the laws, regulations and other requirements”. 4.3.3. 目标 Aims4.3.3.1本司针对各部门建立并保持自身的职业安全卫生目标并形成文件。 We would establish aims of occupation security hygiene in the light of different department and form them into files. 4.3.3.2 本司职

38、业安全卫生目标由人力资源部组织制定,由职业安全卫生管理者代表审核,总经理批准执行。 The aims of the occupation security hygiene are established by the HR Department, audited by the occupation security hygiene management representative and approved by the general manager. 4.3.3.3 本司所设立职业安全卫生目标应与本司职业安全卫生方针保持一致,并包括对消除或减少风险的承诺。 The aims and pol

39、icies of the occupation security hygiene should in accord with each other, meanwhile the occupation security hygiene should includes promises on decline and remove dangers. 4.3.3.4人力资源部在建立和评审职业安全卫生目标时,应考虑法律与其他要求、自身的职业安全卫生危险、技术可行性、财务可行性、运行和经营的实际要求以及相关方的观点。 The HR Department should consider laws and o

40、ther requirements, ones own occupation security hygiene dangers, technical feasibility, finance feasibility, the actual requirements of the operation and management and relevant points while establish and evaluate occupation security hygiene aims. 4.3.3.5人力资源部对所设立的目标不断进行评审,从而确保其与本司的重大职业安全卫生危险、职业安全卫生

41、风险、经营要求的一致性。 The HR Department should evaluate those established aims continuously so as to assure that it could in accord with the great occupation security hygiene dangers, occupation security hygiene dangers and running requirements. 4.3.3.6如果本司重大职业安全卫生危险发生改变或经营战略发生变化时,应由人力资源部及时汇报管理者代表,由人力资源部组织更新

42、和调整。 If there is any occupation security hygiene dangers happen or running stratagem changes, the HR Department should report it to the management representative immediately as well as renew and adjust it. 4.3.3.7每年末,人力资源部应根据本司目标的完成情况评价自身的职业安全卫生绩效,并以此作为下一年度目标设立的基础。 The HR Department should evaluate

43、the achievements of our occupation security hygiene aims at the end of each year and to establish fundament for next year. 4.3.4安全卫生管理方案Security Hygiene Management Projects4.3.4.1 本司制订安全卫生管理方案以实现所确定的职业安全卫生目标。 We establish the security hygiene management project so as to achieve the assured occupatio

44、n security hygiene aims. 4.3.4.2人力资源部负责组织制定本司的职业安全卫生管理方案,管理者代表批准后执行。 The HR Department takes charge of organizing and establishing our occupation security hygiene management projects, which would be implemented after approved by the management representative. 4.3.4.3 职业安全卫生管理方案的内容包括occupation securi

45、ty hygiene management project includes: a. 规定组织的有关职能和各部门实现职业安全卫生目标的职责;To regulate relevant functions of the organization and to let each department fulfill its own occupation security hygiene aims. b. 实现目标的方法和时间表。Ways and timetables of achieving aims 4.3.4.4人力资源部每一年一次对所制定的职业安全卫生管理方案的实施进行监督检查,及时提出调整有

46、关方案建议并随时向职业安全卫生管理者代表汇报。The HR Department should check the implement conditions of the established occupation security hygiene management measure each year as well as provide relevant adjusting projects and advises and report them to the occupation security hygiene representative in time. 4.3.4.5职业安全

47、卫生管理方案应根据本司生产、管理活动的变化而出现的新的重大职业安全卫生危险,建立新的职业安全卫生目标,并进行不断的合理的调整修改。 最高管理者 Top leaderThe new aims of occupation security hygiene management projects should be established and continuously adjusted and modified according to great occupation security hygiene dangers, which happened after the change of ou
